As carers of loved ones, it is important and reassuring to plan ahead and have necessary documentation completed for if and when you may need it. This is also important for you as a carer, in case a family member has to step in to assist.
Join us for a session with ACT Health’s Advance Care Planning team, where we will learn
• what is an advance care plan, the different types and what they legally enable a carer to do on someones behalf;
• what to consider as part of the plan for you or the person you care for;
• how to have a conversation with loved ones about your wishes (tips and tricks);
• how plans are activated;
• and more.
Please arrive 10 minutes prior to the session to allow for Covid-19 screening.
