As carers of loved ones, it is important and reassuring to plan ahead and have necessary documentation completed for if and when you may need it. This is also important for you as a carer, in case a family member has to step in to assist.
Join us for a session with ACT Health’s Advance Care Planning team, where we will learn
- what is an advance care plan, the different types and what they legally enable a carer to do on someones behalf;
- what to consider as part of the plan for you or the person you care for;
- how to have a conversation with loved ones about your wishes (tips and tricks);
- how plans are activated;
- and more.
This activity is open only to carers registered with Carers ACT.
This session will be online only via Teams.

As this session is being live streamed, participants are reminded to arrive before or on time. The session will commence once the facilitator is available and as close to the advertised time.
